As a friendly reminder, a minimum of 2 business days notice is required prior to the appointment for cancellation or change. Otherwise a no-show fee appropriate to the scheduled service will be charged to your account.
Appointment Dates / Times / Locations
Medical appointments are generally available Monday through Friday from 8:30 AM to 4:30 PM. Cosmetic appointments are generally available Monday through Friday from 8:30 AM to 4:00 PM. When requesting an appointment, please indicate which of our five convenient Metroplex locations you would like to be seen in.
When confirming your appointment, we will ask you to provide the following information.
- Patient’s Name
- Patient’s Date of Birth
- Insurance Card (Subscriber or Insured’s Information)
- Pharmacy Name and Phone Number
To decrease the time you have to spend in the waiting room, please take a few minutes now to print and fill out the forms required to assist us in filing your claim accurately and efficiently. Then be sure to bring all three forms: New/Reactivated Patient Information, Patient Medical History, and Patient Notification with you to your appointment. These and other patient forms are available to be downloaded from the Patient Forms page (upper left of this page or top of any page).
