Medical Appointment Scheduling
To request an appointment call 214-987-3376 or click here to schedule an appointment online. As a friendly reminder a minimum of 24 hours notice is required for cancellation or change of appointment.

Cosmetic Appointment Scheduling
To schedule a cosmetic consultation or treatment appointment with one of our Skin Care Specialists, call 214-987-3376As a friendly reminder, a minimum of 48 hours (2 business days) notice is required prior to the appointment for cancellation or change. Otherwise a no-show fee appropriate to the scheduled service will be charged to your account.

Appointment Dates/ Times/ Locations
Medical appointments are generally available Monday through Friday from 8:30 AM to 4:30 PM. Cosmetic appointments are generally available Monday through Friday from 8:30 AM to 4:00 PM. Keep in mind that Dallas Associated Dermatologists has six convenient Metroplex locations. Be sure to check your appointment reminder for the scheduled location.

Required Appointment Information
Whether by online request or phone call, please have the following information available when scheduling your appointment and bring it with you to the appointment.

  • Patient’s Name
  • Patient’s Date of Birth
  • Insurance Card (Subscriber or Insured’s Information)
  • Pharmacy Name and Phone Number

To decrease the time you have to spend in the waiting room, please take a few minutes now to print and fill out the forms required to assist us in filing your claim accurately and efficiently. Then be sure to bring all three forms: New/Reactivated Patient Information, Patient Medical History, and Patient Notification with you to your appointment. These and other patient forms are available to be downloaded from the Patient Forms page (upper left of this page or top of any page).

Cancellations and Rescheduling Policy
Please contact us at your earliest convenience, if you are unable to make your scheduled appointment. This courtesy will allow our office to make the appointment available for other patients. We thank you, in advance, for your cooperation.

Payment Options
Payment is expected at the time services are rendered at Dallas Associated Dermatologists for medical and cosmetic services and products. For your convenience, we accept many insurance plans and offer an array of other payment options, including cash and check, most major credit cards, and CareCredit®.

Most treatments for skin diseases are covered by insurance to some degree. However, many cosmetic procedures are not covered under many medical healthcare plans. And, to make it more complex, a procedure that is categorized as medial dermatology and covered under one plan, may be considered cosmetic dermatology and not covered under another, even though it is actually the same service being provided. Please refer to your individual plan for such coverage details.


Although our practice is contracted as a provider for many major health insurance carriers in the Metroplex, it is the patient’s responsibility to know what insurance company currently provides his or her coverage and the details of that coverage – including which treatments and procedures are covered within that plan. Employers often change plans annually, so we will ask to see your insurance card/information at each visit. This step helps to ensure that our records are accurate and your claim may be filed and paid with as little inconvenience to you as possible.

Please, have the following Subscriber or Insured’s information available when you schedule your appointment AND when you check in for your appointment:

  • Name
  • Date of Birth
  • ID# or Social Security #
Policy Information:
  • Coverage effective dates
  • Individual policy or employer/group policy?
If it’s an Employer/Company Policy:
  • Employer/Group Name
  • Employer/Group Number
  • Health Savings Accounts (HSAs)

If you have not met your deductible, we will still bill your insurance carrier for your visit charges; however, we will also collect any outstanding deductible or co-insurance that is due at the time of your visit. Once we receive our Explanation of Benefits (EOB) statement from your insurance carrier, we will bill you for any remaining balance.
