We Are Paperless
We all know that forms and paperwork are an important part of today’s healthcare environment. We have worked hard to make the process as easy as possible for all of our patients. At Dallas Associated Dermatologist, all patient intake is done electronically (saving time and trees)! You can even pay your co-pay and/or any outstanding balance from your own personal device.

Scheduling Your Visit
Insurance
Our practice is contracted as a provider for many major health insurance carriers. Although, we are not always contracted with every plan each insurance carrier offers.
Most treatments for skin disease are covered by insurance to some degree. However, many cosmetic procedures are not covered under many medical healthcare plans.We advise all patients to refer to their individual plans to determine coverage for procedures they wish to have.
Insurance plans often change annually as well, so we will ask to see your insurance information at each visit. This can help to ensure that our records are accurate and that you have proper coverage, so your claim can be filed and paid with as little inconvenience to you as possible.
Please, have the following Subscriber or Insured’s information available when you schedule your appointment AND when you check-in for your appointment:
- Name
- Date of Birth
- ID# or Social Security #
Policy Information:
- Coverage effective dates
- Individual policy or employer/group policy
If it’s an Employer/Company Policy:
- Employer/Group Name
- Employer/Group Number
- Health Savings Accounts (HSAs)
If you have not met your deductible, we will still bill your insurance carrier for your visit charges; however, we will also collect any outstanding deductible or co-insurance that is due at the time of your visit. Once we receive our Explanation of Benefits (EOB) statement from your insurance carrier, we will bill you for any remaining balance.

For your convenience, we accept many insurance plans and offer an array of payment options including:

Before Your Visit
Prior to your visit, you will receive an appointment confirmation text or email from us. If this is your first visit, you will also be prompted to complete your new patient intake forms. Clicking the link to confirm your appointment will automatically lead you to complete your patient registration and prompt you through the entire process. It’s easy to fill out, and we will remember your answers for your next visit, so you only have to make necessary changes in the future.
Registering before your visit helps us serve you better and faster when you come into the office. We understand that sometimes it can be difficult to fill out forms on a mobile device or computer, which is why we do offer the option to do it on our iPads in the office. Please arrive at least 15 minutes early to fill out your pre-visit information on our electronic iPads if you cannot complete it before your appointment.
Financing with CareCredit®
For cosmetic patients wanting the ability to pay over time with low minimum monthly payments, Dallas Associated Dermatologists offers CareCredit®.
A leader in the patient financing industry, CareCredit® provides a combination of short-term interest-free and longer-term low-interest plans. Transaction minimums and some restrictions apply so visit their website for complete details.

Cancellation Policy
Dallas Associated Dermatologists reserves the right to charge a fee of $50 or loss of deposit if a patient does not cancel his/her appointment before 24 hours of appointment time. Appointments scheduled with Aesthetics require a 48 hours notice of cancellation and will be charged a $100 fee for failure to provide 48 hours notice of cancellation. Administrative fees incurred for failure to provide notice or cancellation are not billable to insurance or any other third-party payor.
